Summary The LTC Charles S. Kettles VA Medical Center is seeking a Part-Time Registered Nurse Community Care Coordinator within Chief of Staff (COS) service. The VA Community Care Coordinator facilitates and coordinates services between providers of the VAAAHS and non-VA providers in the local community for outpatient and inpatient care that cannot be provided by the VA or cannot be provided due to access. Responsibilities Major Duties Include, but are not limited to: Provides direct staff support as well as oversight to the VA Community Care program in planning, designing, integrating, implementing, modifying, and evaluating the effectiveness of program components and services to improve the quality of patient care and the appropriateness and timeliness of services provided. Facilitates and coordinates services between providers of the VAAAHS and non-VA providers in the local community for outpatient and inpatient care that cannot be provided by the VA or cannot be provided due to access. Processes consults utilizing clinical judgment, VACC processes, and VA guidelines and resources. Works collaboratively with fellow RN coordinators in Community Care, along with supportive medical staff to ensure timely care occurred in the community. Care coordination of episodes of care in the community will be done within the guidelines set forth by the office of community care. Functions independently within his/her scope of practice and demonstrates decision making capacity. Possesses thorough knowledge of InterQual criteria, utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. Applies the nursing process to systems or processes at the unit/team/work group level to improve care. Accurately documents in the medical record patient findings, assessments, and care provided. Demonstrates consistent clinical competence in providing nursing care. Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes. Demonstrates grade-appropriate and consistent critical thinking skills. Supports and enhances patient self-determination. Serves as a resource for patients and staff in addressing ethical issues. Identifies and assesses resource utilization and safety issues, taking appropriate action. Acquires knowledge and skills to maintain expertise in area of practice. Evaluates practice of self and others using professional standards, relevant statutes, and regulations. Uses established business rules for Enrollment and Registration, Scheduling, Consult Management, Use of the VistA Electronic Wait List (EWL), Handling No-Shows, Patient Cancellations, and Clinic Cancellations as applicable to job. Uses group process to identify, analyze, and resolve care problems. Initiates/participates in quality improvement activities that result in approved outcomes. Uses body of research to validate and/or change work group practice. Must be poised, articulate, adaptive, and respectful in communicating with and caring for people of varied educational, ethnic, religious, and cultural backgrounds. Obtains translation services for patient or families with limited English language proficiency or who need sign language services. Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, federal regulations, VA statutes and policy, and VHA policy. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U. S. C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program.
