The Registered Nurse is responsible for performing the duties of a registered nurse in the primary care facility. They collaborate and support the team members to ensure a patient-focused approach to care. The Registered Nurse assesses the physical and psychosocial status of patients by means of interview, health history and physical examinations.

As part of the essential functions of this role, the Registered Nurse:

Facilitates patient continuity of care with the healthcare team in an efficient and timely manner.

Collaborate and support the team members to ensure a patient-focused approach to care.

Assesses patient needs.

Renders health education regarding different treatment options.

Provides patients with information on prescribed therapies and discusses expectations.

Completes appropriate and concise documentation.

Assists with patient medication refills and complying with NYS PMP (I-STOP) regulations.

Assists with rooming patients as needed.

Administers medication and provides treatment prescribed by provider.

Performs immunizations and injections.

Performs continuous collection of health data about the health status of patients.

Works in collaboration with other RNs to ensure clinic duties, tasks and coverage are completed.

Actively strives to improve patient service delivery through participation in Quality Improvement activities.

Collaborates with other health disciplines to ensure continuity of care.

Conducts patient education to increase knowledge of disease process, terminology, medication, assessment tools (routine labs) and treatment options using clinician recommendations, educational tapes, booklets, Internet resources, HIV magazines and journals.

Makes appropriate referrals to other healthcare providers and consults with, or refers patients to, providers as per treatment protocols.

Other duties as requested.

Qualified candidate will have a current license to practice as a Registered Nurse in New York State with a current BCLS certification and one (1) year of experience in a medical office setting, preferably in an HIV/AIDS or other infectious disease practice. Candidate must have demonstrated phlebotomy skills. Candidate must be able to communicate clearly and professionally in writing and verbally and have familiarity with computers and MS software (Outlook, Word) preferred. Candidate must have familiarity with electronic charting system (Medent) preferred. Sensitivity toward cultural diversity, people living with HIV/AIDS and lifestyle and addiction issues essential

Required license/certification: RN in New York State, BCLS

Required experience: Medical office setting, HIV/AIDS or other infectious disease practice (1 year). Phlebotomy skills preferred.
