Description

The health services that became theAnthony L Jordan Health Corporation () (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY () and in Canandaigua, N. Y.

Jordan Health is now seeking aReferral Specialist/QIwho, under the guidance of the Director of Quality Assurance, is primarily responsible for serving as the critical link between the patient and the care delivered by Jordan Health clinical and service staff. TheReferral Specialist/QIis responsible for pre-visit planning, referral processing and tracking, closing the referral loop, transportation forms and outbound fax request by clinical teams. We focus on having a connection with the patient by offering them support through Motivational Interviewing to get their prevention screening and specialty care done. This position supports Quality Improvement by tracking persons who are overdue for prevention care screening and setting up the screenings. We will focus on new modes of technology to connect with our patients, texting, messaging within E Clinical Works as examples.

Requirements

TheReferrals Specialist/QIwill ensure that:

· Jordan protects and values compliance with all regulatory and safety requirements at all times by attending all mandatory trainings offered or promoted by Jordan Health.

Education And Experience Required:

· High School diploma or equivalent is required.

· One year of directly related experience in a health care setting is strongly preferred.

· Two years working in a position requiring customer interaction, demonstrating the ability to handle competing priorities.

· Computer or business school training is highly desirable.

· Bilingual in Spanish preferred.

Licenses And Certifications:

· For native English Speakers who present as bilingual in Spanish, proof of proficiency in Spanish (written and speaking) required prior to hire.

· For native Spanish Speakers who present as bilingual in English, proof of proficiency in English (written and speaking) required prior to hire.

Special Skills, Knowledge Required:

Must be able to demonstrate competency in:

· MS Office Applications (Word, Excel, Outlook).

· eClinicalWorks or other electronic health record preferred.
