Summary You will serve as the Program Applications Specialist (Primary Care Management Module PCMM) Coordinator for the Primary/Ambulatory Care service located at the Mann-Grandstaff VA Medical Center. As coordinator for the Primary Care Teams within the Primary/Ambulatory Care Service assigned under the Associate Chief of Staff and works in collaboration with the Clinic Director, Clinical Coordinator, Providers, Nurses, Clerks and the rest of the clinic staff. Responsibilities The duties of the position include but are not limited to the following: Maintain the PCMM database for all of primary care throughout the hospital service area Work in collaboration with the team leaders, Clinical Managers, Providers, Nurses, Clerks, and the rest of the clinic staff, and is responsible for maintaining the PCMM database for all of Primary Care throughout the hospital service area Assist, or provide training, implementation, monitoring and evaluation of all administrative procedures related to PCMM operations as well as work closely with the clerks in all primary care settings serving as a PCMM resource person, and will certify PCMM application users to ensure competency Ensures maximum efficiency of the Primary Care clinic operations through appropriate utilization of resources; support to and involvement in, optimal management information system data gathering; and analysis, validation and presentation Builds, maintains, and monitors clinics Provides assistance and technical level support from within the Primary Care Service or a comparable setting Maintains full responsibility and accountability for PCMM in the Primary Care setting Recommendations and suggestions to management are necessary to assist them with ensuring performance monitors/measures are being met Performs work in support of general management and/or program analysis functions in which knowledge of the purpose, operation, methodology and techniques characteristic of specific management/program analysis functions required; specifically for the Primary Care Management Module, which includes physicians' panels and the next available appointment Provides specific reports regarding timeliness and quality of care indicating panel sizes, clinic activity and workload patterns arid reports variances in clinic operations to Primary Care Management Enters Primary Care team information into PCMM system, enrolls patients into the Primary Care teams, and assigns patients to appropriate provider within PCMM systems Assigns patients to Primary Care teams, removes patients from the database when discharged from Primary Care, maintains accurate enrollment data, identifies and corrects discrepancies in the database system, and discharges patients from multiple or inappropriate specialty and consult clinics Monitor workload reports in VISTA, including, but not limited to, those concerned with incomplete encounters, encounter errors and encounter action required; will inform supervisory personnel when reports indicate any situation that could cause Ambulatory Care workload to go uncounted or un-transmitted to the Austin Automation Center Responsible for obtaining and maintaining accurate and timely information on provider-specific vesting compliance in Primary Care Works with Team Leaders and Clinic Managers to formulate and conduct training and orientation for all providers and clerks involved with PCMM and provides expertise and advice to all providers and clerks involved with PCMM Troubleshoots and coordinates processes with all providers and clerks involved with PCMM Conducts portion of organizational surveys in a wide variety of organizational activities particularly in Primary Care Analyzes workflow to determine potential problem areas and makes recommendations to correct these areas and to improve coordination between the various physicians, teams and clinics Identifies and recommends ways of eliminating, combining, simplifying or improving procedures and processes Performs clerical work in support of the care and treatment given to patients such as receiving patients in the Primary Care clinics or maintaining patient files and visits in the various software packages, i. e., PCMM, for the medical facility Manages administrative functions associated with the PCMM Primary Care program for the facility Assists professional staff with and handles administrative functions of the office Work Schedule: Monday-Friday; 7:30 am-4:00pm Telework: 1-2 days per pay period authorized.
