Summary The Licensed Practical Nurse is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN is a member of the larger team and reports to the primary care manager. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. Responsibilities Duties include but not limited to: Makes observations and thoroughly documents care and the patient's response. Appropriately seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention. The LPN performs, reports, and records assigned nursing care in the Primary Care Clinics. Adheres to standards of practice and safe technique in preparation and administration of medication. Demonstrates understanding of age- and population-specific characteristics that affect the health care needs of the Veteran. The nurse will provide culture specific care to patients in Caucasian, African-American, Hispanic, Asian, Native American and all other cultures. Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate. Based on the patient's goals, provides patient and family health education, to include self-management, prevention and wellness. Interacts and communicates appropriately with patients and family members. Demonstrates professional behavior (i. e., respect, civility, ability to accept constructive criticism), effective communication and interpersonal skills. Adheres to principles of infection control, safety, and universal precautions. Incumbent possesses knowledge of proper handling and pre-cleaning of reusable medical equipment. Demonstrates competence utilizing special equipment needed to perform job duties. Follows policy and procedures related to fire and disaster drills, safety and security measures. Demonstrates initiative in assisting the Teamlet and larger Teams. Assists in the PACT orientation and competency validation of students, peers and clerical staff. Maintains a work environment that honors diversity. Serves as a resource and advocate in the protection of patient and family rights. Completes mandatory reviews, including motivational interviewing, in order to promote patient self management toward individualized life plan of care. Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles. Promotes patient and personnel safety. Demonstrates competence utilization of Safe Patient Handing equipment. Technically proficient in initiating, performing and completing assigned duties based on the PACT model while providing care to variable patient populations. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.· Must be a graduate of practical or vocational nursing approved by the appropriate State accrediting agency and/or the National League for Nursing at the time the program was completed by the applicant.· Must be proficient in spoken and written English as required by 38 U. S. C. 7402(d) and 7407(d).· Must have a full, current, active and unrestricted license as a graduate licensed practical or vocational nurse in a State, Territory, or Commonwealth (i. e., Puerto Rico) of the United States or District of Columbia.· Must pass a pre-employment examination if selected. Preferred Experience:· A minimum of 3 years of successful clinical nursing practice as a Licensed Vocational Nurse· Broad knowledge base with experience in a variety of patient care settings and with diverse patient population.· Demonstrated skill in performing multiple tasks, managing assignments, and expediting oral and written communications· Demonstrated ability to interact with others effectively and appropriately, presenting a professional image of the service and the VA. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e. g., Peace Corps, AmeriCorps) and other organizations (e. g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. GRADE DETERMINATIONS:GS-3: None beyond the basic requirements. LPNs at this grade level are expected to perform all duties within the scope of their license (e. g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties Employees may be appointed at or advanced to GS-4 if they meet any of the criteria below:· Six months of qualifying experience as an LPN or LVN; or Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or Graduation from an approved school of at least 24 months Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.· Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/LVNs and/or NAs/HTs in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes.· Demonstrated
