Perinatal Navigators Essential Job Duties:
SNF experience preferred
Will work directly with post-acute NP to coordinate care of ACO and Medicare patients
Will follow up on labs, follow up appointments, plan of care, and advanced directives
Will also work with post-acute coordinate and social worker.
Participate in nursing home huddles.
Review discharges to identify those high risk for readmission and communicate findings to NP.
Education, Training & Experience
Must have active IN RN license
Must have graduated from an accredited nursing program
Must have active BLS certification
