Responsibilities:
Utilize the nursing process to plan, implement, and evaluate patient care for clients receiving contracted services.
Initiate and revise care plans as needed, based on patient status and needs.
Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and goals.
Respond to changes in patient condition and report to the appropriate physician or supervisor.
Administer medications and treatments as prescribed, and monitor/document their effectiveness.
Review care plans and protocols, identifying areas for healthcare teaching, care coordination, goal setting, and discharge planning.
Requirements:
Current LPN license for the state of practice.
Current Health Certificate (per facility RN contract or state regulation).
Current PPD or Chest X-Ray.
Current BLS card.
One year of prior LPN experience preferred.
