This position requires the selective applicant to work weekends and evening hours as a condition of employment.

Your Specific Responsibilities:

This highly responsible position will coordinate the local Healthy Communities Florida, a public-private collaboration bringing together state agencies, not for profit organizations, businesses, and entire communities to help Florida's children and adults make choices about healthy eating, active living, and overall wellness. This incumbent will also be responsible for organization and follow-through of agency community outreaches. Included in this responsibility is the Food Pantry, sponsored by the Sharing Center. This position is also responsible for creating, monitoring, and following thru on referrals for DOH-Seminole clients. This position will require some evening and weekend activities outside of regular work hours. Furthermore, the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness, and Excellence in all aspects of their work.

Healthy Communities Florida program consists of the following expectations:

Responsible for coordination of the planning, implementation, and evaluation of deliverables in the Healthy Communities annual work plan and county health department strategic plan. Prepares semi-annual reports, attends virtual conference calls, meetings, and trainings. Promotes county-wide marketing of health campaigns and messaging from the Florida Department of Health.

Community Outreach expectations include, but are not limited to the following:

Receive requests from partners and community-based organizations and document the request within our project management database. Send outreach information to all DOH-Seminole staff and communicate with the organizer and staff that will attend the outreach about any pertinent updates. Organize flyers, brochures, and any other materials for the outreach and stage it for packing into designated vehicle. Attend outreaches as a representative of the Office of Health Promotion and Education. Ensure that data is received from all staff that attended the outreach and update the project management database accordingly.

Food Pantry: Coordinate with the Sharing Center to ensure there are enough bags of food for clients, Organize/set-up the Auditorium for the Food Pantry, Assist the Sharing Center on the day of the Food Pantry, Monitor the use of the iPads to ensure that clients new to the pantry complete the Smart sheet survey, Recruit volunteers to assist with the Food Pantry, Remain in the Food Pantry until the designated number of clients have been served.

Referral Specialist expectations include, but are not limited to the following:

Utilizes our online referral platform to enter client information, creates referrals to organizations based on the client's needs, assists other departments with referrals for clients, and attends monthly virtual calls with online referral platform staff.

Strong attention to detail and accuracy in data management.

Strong interpersonal and communication skills.

Ability to work independently and as part of a team

Qualifications:

Florida Department of Health is required to work before, during, and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelter or performing other emergency duties including but not limited to, response to or threats involving any disaster or threat of disaster man-made or natural.

Preferred -

Bilingual in Spanish and English preferred.
