Job Title: Nurse Care ManagerJob Description

We are seeking a dedicated and compassionate Nurse Care Manager for a 100% remote position. You will be responsible for developing, assessing, and facilitating complex care management activities for members primarily with physical needs to ensure high quality, cost-effective healthcare outcomes. This includes creating personalized care plans and providing education for members and their families. You will be expected to evaluate the needs of the member, barriers to accessing the appropriate care, social determinants of health needs, focusing on what the member identifies as priority, and recommending or facilitating the plan for the best outcome. The role entails coordinating between the member, caregivers, and the care provider team to ensure members are receiving adequate and appropriate person-centered care or services. This role will require ongoing follow up and monitoring of member status, change in condition, and progress towards care plan goals. You will also be responsible for providing resource support to members and care managers for local resources for various services based on service assessment and plans, as appropriate.

This is a contract position with a duration of 13 Week(s).

Work Site

This is a fully remote position.

Work Environment

This is a 100% remote position. The candidate must be able to work PST hours, and it is preferable if they also live in the PST timezone. The schedule is not flexible. The start date is flexible, with an initial term ending on October 25, 2024, with a potential to extend. Please note that the performance expectations for this role include a caseload target of 75-80, case duration of 90 days, and addressing 90% of Care Gaps.
