Program Supervision
Duties and Responsibilities:
Responsible for supervision of Pregnancy Care Management and CPS services
Responsible for providing care management to a high risk patient population
Provides Pregnancy Care management services to patients in private sector, as well as at the health Department.
Performs pregnancy assessments to identify needs and services
Develops plans to include all services needed including those provided by other community agencies.
Utilize In-depth and advanced casework skills to counsel patients.
Contract Details:
13 week contract with high probability of extension
40 hours weekly. 8a - 5p. Monday - Friday
Payrates available for local and travel
Requirements:
Minimum of 1 year of relevant experience
AHA BLS Certification
Actively Licensed RN for North Caroline or Multistate
Dedication to fulfill a 13 week contract term
