Job Description

An employer in a Southern California is seeking 2 LVNs. Candidates must be 50 miles from Ontario and must be open to going into the office twice a year. Hours are 8-5pm Monday through Friday. Day to day includes:

Creating care plans, calling members, working with the coordinators to make the phone calls, and delegating tasks to the unlicensed person.

The LVNs will be tasked with a partnered RN and the RN will sign off on the case. The LVN, RN and Case Manager are working as a team.

Skills and Requirements

Current, unrestricted LPN/LVN licensure

3 or more years of Clinical LPN/LVN experience, resulting in excellent clinical skills and judgment

(Prefer case management experience, inpatient, outpatient or managed care)

Strong inbound and outbound telephonic skills

Intermediate computer skills (Microsoft Word, Outlook and Internet) with the ability to navigate a Windows environment and the ability to create, edit, save and send documents utilizing Microsoft Word.

Understanding of change and ability to move members along the continuum of change

Intermediate or expert level of proficiency with being able to assess and identify needs and being able to influence and motivate

Conflict management skills

Able to work under pressure and appropriately prioritize responsibilities Coaching and/or decision support experience

Managed care experience null
