**Job Overview**

The Community Care LVN supports the Chief Medical Officer by conducting health assessments, providing health information, and making referrals through home visits and telephonic outreach. The role embodies values like trust, empathy, and community-mindedness.

**Responsibilities**

Build supportive relationships with individuals and families.

Conduct home visits for physical exams under remote clinician supervision.

Provide follow-up, motivational interviewing, and goal-setting.

Perform physical exams, point-of-care testing, and administer medical interventions.

Manage clinical supplies and inventory.

Facilitate telehealth visits.

Complete forms and screening tools.

Follow up via phone, home visits, and facility visits.

Coordinate care with medical providers and other clinical personnel.

Stay informed about community resources.

Communicate individual status to the clinical team.

Act as an advocate and liaison with community service agencies.

Record care information in EMR within 24 hours.

Assist with setting health goals and attending appointments.

Provide referrals to community agencies.

Help with transportation resources and appointment reminders.

Maintain good working relations and HIPAA compliance.

Attend meetings and training sessions.

Perform other duties as assigned.

**Qualifications**

**Education:** Licensed Vocational Nurse from a US-accredited facility.

**Experience:** 1 year in a health services environment. Community Health Worker experience preferred. Familiarity with primary care practices and home care experience are pluses.

**Skills:** Proficiency in Microsoft Office. Bilingual in Spanish and English preferred. Strong communication and relationship management skills.

**Physical Demands**

Typical office environment tasks with frequent travel by car or public transportation.

Ability to carry medical supplies and equipment.

**Work Environment**

Regular travel to homes, physician offices, hospitals, facilities, and community partners.

Reliable transportation required. Moderate noise levels.

**Compensation:** $43K-56K

**Location:** Field-based, involving visits to homes, hospitals, facilities, and main offices.
