What is the client know for? Selinsgrove Center is a state-owned intermediate care facility located on 254 acres of land, centrally located in Snyder County, just outside the borough of Selinsgrove. The Center serves 250 people in 19 different living areas. The age range of the people living at the center is from 23 to 95 years. The vast majority of people have called Selinsgrove Center their home for over 40 years. All 900 employees play an integral part of the multi-disciplinary team which focuses on a person-centered approach for services and supports that we provide. This holistic staff approach is vital to the success of the Center and our dedication to supporting people with intellectual disabilities in achieving their personal life goals in an effort to lead an everyday life.

The people who reside at Selinsgrove Center have varying levels of intellectual disabilities which require extensive supports with daily living, social skills, medical care, and vocational training. The Center provides 24-hour a day medical services, as well as, occupational and physical therapy, psychiatric, speech and hearing, recreational and social work services. Selinsgrove Center is family-oriented and strives to include family members in all aspects of their loved one's life. Family members are encouraged to participate and assist in devising an annual comprehensive plan to support their loved one, as well as frequent visits and attendance at various recreational events like the Family Picnic, Spring Fest, and the Memorial Day Bash.

Selinsgrove Center is actively involved in the community. Local community groups are encouraged to volunteer at the Center and many of the people who live at the Center are members of local civic groups. The Center is a member of the Greater Susquehanna Valley Chamber of Commerce. There is a Recycling Center on-campus that provides convenience for businesses in the Selinsgrove area and employment opportunities for the individuals who live at the Center.

Maintains patient confidentiality of all clients

Performs duties within scope of practice, as assigned, in an accurate and timely manner

Adheres to facility/client policy, procedures and protocols at all times

Assists doctors, nurses, and other healthcare providers with treatments, procedures, and tests being performed in a variety of patient care settings

Safeguards the client/patient by observing appropriate infection control procedures including but not limited to universal precautions

Notifies the supervising nurse of any abnormal findings or conditions

Demonstrates flexibility and adaptability in meeting the needs of all facilities

Minimum Requirements:

Prefer one year experience as a certified nursing assistant within the last three years

Graduate from certified nursing assistant program with subsequent documentation as a certified nursing assistant per state requirements with proof of verification as being an active certification and in good-standing

Knowledge of medical terminology and knowledge of clerical functions

Complies with all relevant professional standards of practice

Participation and completion of Amergis' Competency program when applicable

Current CPR if applicable

TB Questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or state regulation)

Must meet all federal, state and local requirements

Successful completion of new hire training as applicable to job site

Understand patient confidentiality and HIPAA requirements

Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

Computer proficiency required

Must be at least 18 years of age

The Long Term Care RN assumes responsibility and accountability for the delivery of care for patients in a Long Term Care and/or Assisted Living Facilities. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision, as well as critical thinking and performance abilities in the coordination of patient care. Additionally, this individual performs according to nursing standards and is accountable for managing patient care and assisting others in the management of patient care.

Essential Duties and Responsibilities:

Utilizes the nursing process to assess, plan, implement and evaluate patient care in a Long Term Care/Assisted Living Facility

Performs focused interviews to identify specific needs

Assesses signs and symptoms indicating physiologic and psychological changes in the patient's condition

Collects, analyzes, and interprets data and information from healthcare team members and documents actual and/or potential nursing diagnoses

Documents the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions as well as responses to interventions

Performs interventions according to identified priorities, plan of care, and the facility policies and patient outcome standard

Provides a safe, comfortable and therapeutic patient environment

Initiates and evaluates patient and/or family education

Uses clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit including establishing priorities

Monitors safety and effectiveness of the environment and equipment

Participates in activities to facilitate good interpersonal communications with the chosen facility
