Description
Under the supervision of a Registered Nurse, the LPN/LVN (Licensed Practical/Vocational Nurse) - Home Health provides skilled one-on-one nursing care to patients in their home and accompanies them when indicated to appointments, etc.
Essential Duties and Responsibilities:
Documents each shift or visit to reflect accurately the provision of skilled nursing care rendered in accordance with physician's orders.
Changes in patient condition are identified and documented with resolution on the appropriate clinical form. Director/Supervisor notification if applicable
Physician notification to obtain necessary orders for intervention(s) per state regulations
Performs specific treatments and medication administration in accordance with physician orders and home health agency's protocols
Documentation reflects patient collaboration and participation in their plan of care
Requests for modification of orders and additional orders are made based on the patient's clinical status and skilled nursing judgment
All physician collaboration/coordination of care is documented in the medical record and communicated as needed
Provides patient and family education as appropriate
Attends case conferences, team meetings and education in-services as required
Minimum Requirements:
Graduate of an accredited practical or vocational nursing program
Currently licensed as a Licensed Practical/Vocational Nurse in the state in which the LPN/LVN will practice
One year prior professional nursing experience, home care experience preferred
Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
Understand patient confidentiality and HIPAA requirements
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
Computer proficiency required
Must be at least 18 years of age
