Description
General Summary
Under the supervision of the registered nurse, utilizes the knowledge and practical experiences of a licensed practical nurse to provide direct patient/family care in the home. Performs basic therapeutic and preventative nursing procedures.
In collaboration with the Intake and home health operations team, facilitates a timely and high quality initiation of home health services. Works with the Intake team to screen home health referrals for appropriateness. Works with referral sources and other healthcare members to obtain the necessary information including the reason for the home health referral, the primary medical diagnosis and the certifying physician to ensure the home health clinicians have the needed information and the agency maintains compliance with regulations.
EDUCATION/EXPERIENCE REQUIRED
Successful completion of a licensed practical nursing program accredited by the National League of Nursing. A minimum of one (1) year hands on clinical LPN experience required; prior home health care experience preferred. The ability to effectively communicate with patients, families and health care personnel. Strong interpersonal and customer service skills required in order to interact effectively with individuals from various cultural and socio-economic backgrounds. Must have reliable transportation; possess a current State of Michigan driver's license; and have a satisfactory driving record. Must be able to lift, carry, transport and move clients, if needed.
CERTIFICATIONS/LICENSURES REQUIRED
Current licensure by the State of Michigan as a licensed practical nurse (LPN). NAPNES certification or successful completion of pharmacology exam.
