BASIC FUNCTION:

The Licensed Practical Nurse II under the supervision of a registered professional nurse provides supportive and restorative care including basic patient care responsibilities. Activities include, but are not limited to, basic hygiene and comfort needs, nutrition and fluid balance including tube feeding and monitoring IV fluids (including the addition of pre-mixed IV meds), medication administration consistent with scope of practice and regulatory requirements, reinforcement of the established teaching plan and the contribution of information for the planning of care.

SECTION II

EDUCATION AND EXPERIENCE REQUIREMENTS:

Graduation from an accredited LPN Program.

Current licensure in New York State or eligibility for limited permit to practice.

Documentation of successful completion of pharmacology course required, either through basic program or other approved coursework.

Current BLS is required.

No previous experience required.

PHYSICAL REQUIREMENTS:

Should be able to push/pull, lift/move 35 lb., be able to perform moderately difficult manual manipulations such as using a keyboard, writing, and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to be stationary at a workstation for a prolonged period time in addition to being able to squat or be mobile for a reasonable length of time and distance. Communication requirements include the ability to comprehend the spoken English language in addition to being able to communicate and read the English language.

PERFORMANCE STANDARDS AND RELATED MEASUREMENT CRITERIA:

Performance Standards Measurement Criteria

A.

The Licensed Practical Nurse II will participate in data collection process and report findings to the RN to aid in ongoing development of patient plan of care.

1.

Interviews patient and family to collect pertinent data regarding current illness.

2.

Monitors patient as directed by RN. Communicates findings in a timely manner.

3.

Accurately obtains and records vital signs as ordered, and reports significant variations in a timely manner.

4.

Demonstrates knowledge of age-specific ranges and techniques to obtain vital signs (for unit assigned).

5.

Provides information on patient progress to RN in order to have input into plan of care revision.

B.

The Licensed Practical Nurse II will implement the established plan of care for patient hygiene.

1.

Assists the patient in all ADL including bathing, skin care, feeding, toilet, grooming, dressing and ambulation. Uses age-specific techniques as indicated.

2.

Monitors when appropriate, the healing process of wounds including prescribed dressing changes using correct aseptic technique.

C.

The Licensed Practical Nurse II will monitor patient nutrition and fluid balance.

1.

Measures and records intake of all fluids including intravenous admixtures, oral, diet and tube feedings.

2.

After completion of orientation/clinical validation, administers intravenous solutions observing the following guidelines: a) initiates unmedicated IV solution for peripheral lines, b) replaces bottle/bags for peripheral lines with medicated solutions as outlined in nursing policies, c) changes dressing over IV site prn, d) monitors IV flow rates, e) changes IV tubing, f) discontinues peripheral IV lines, g) trouble shoots flow control devices.

D.

The Licensed Practical Nurse II will provide patients with safe environ-ment.

1.

Monitors patient safety as outlined in PCP and hospital policy.

2.

Monitors patient's skin condition as outlined in the hospital's policies.

3.

Monitors patient in restraint as outlined in hospital policy.

4.

Uses age-appropriate safety measures as indicated to maintain safe patient environment.

5.

Moves and positions patients using correct body mechanics to avoid injury to self or patient.

6.

Operates transportation equipment safely when necessary.

7.

Initiates and maintains CPR until relieved by the Code Blue Team.

8.

Maintains/checks emergency equip-ment on unit as assigned.

E.

The Licensed Practical Nurse II provides emotional support to patient and family.

1.

Reinforces patient teaching as outlined in plan. Uses age-appropriate techniques as indicated.

2.

Explains procedures, which she (he) is administering and refers to appropriate source for more complex procedures.

3.

Communicates in an appropriate man-ner based upon the age and condition of the patient.

4.

Demonstrates respect for the spiritual and cultural beliefs of patients and families.

F.

The Licensed Practical Nurse II will be proficient in use of Ellis Hospital information system.

1.

Demonstrates ability to input infor-mation accurately and retrieve infor-mation as needed.

2.

Documents accurately on appropriate flow sheets in medical record using paper and online methods.

G.

The Licensed Practical Nurse II will demonstrate professional growth in nursing practice. Will demonstrate knowledge of age-specific care for populations as outlined in assigned unit's scope of service.

1.

Functions within the limits of the New York State Practice Act and hospital and nursing service policies and procedures.

2.

Demonstrates skill in knowledge of lifespan development, and the ability to obtain and interpret information regarding patient needs.

3.

Assists in identifying own learning needs and participates in programs and activities to meet these learning needs. Maintains record of own in-service program attendance and/or completion including outside pro-grams, activities and mandatory in-service requirements.

4.

Demonstrates continuous effort to improve skills by participating in unit conferences, asking pertinent ques-tions, and taking initiative to learn in daily activities.

5.

Participates in efforts to continuously improve unit operations.

6.

Recognizes job relationships and functions effectively within the organi-zational structure.

H.
