The Licensed Practical Nurse II (LPN) practicing in Medical Oncology Outpatient office represents and contributes to a multidisciplinary team approach with the focus on the acute ambulatory needs of the oncology or hematology patient. The LPN works collaboratively within the team to provide safe, therapeutic, patient-centric, culturally competent care. The LPN works under the general supervision of a Physician, Advanced Practice Provider (APP) or Registered Nurse to systematically and continuously collect data related to the patients' health status. Through the provision of basic patient care, the LPN II participates in the restorative, health maintenance and wellness plan defined by a multidisciplinary team.

EDUCATION AND EXPERIENCE REQUIREMENTS:

The applicant must be a graduate of an accredited school of Nursing; license to practice as a Licensed Practical Nurse in New York State required.

Current BLS required.

LPNs are responsible to maintain a current level of competence through education and nursing process.

Previous patient care experience is required, Urgent Care experience preferred.

Integral components consist of strong critical thinking with an emphasis on collaborative team behaviors.

PHYSICAL REQUIREMENTS:

The position is located indoors under normal working conditions. Should be able to push/pull, lift/move 35 lb., be able to perform moderately difficult manual manipulations such as using a keyboard, writing, and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to sit at a computer terminal or workstation for a prolonged period in addition to being able to squat, stand and walk for a reasonable length of time and distance. Sensory requirements include the ability to articulate and comprehend the spoken English language in addition to being able to read the English language.

RESPONSIBILITIES OF THE POSITION:

Exhibits professional behaviors and skills in the ambulatory environment.

Adheres to the American Nurses Association (ANA) Code of Ethics and Nursing Scope and Standards of Practice, as well as the New York State Nurse Practice Act

LPN Uses two identifiers to verify correct patient before performing any activity or procedure on or with the patient. Follows policy guidelines and procedures which require full name and patient date of birth as positive identifiers.

Chart prep including making appointments, obtaining medical records on patients, and scanning them into the patients' chart.

Care coordination inclusive of following up on provider orders, obtaining authorizations for testing or treatment, obtaining test results, coordinating care with other providers and following up on patient needs.

Delivers nursing care within the NYS scope of practice for LPN's including medication administration under the guidance of Registered Nurses.

Assist all staff in the practice as needed.

Intake components include, but are not limited to vital signs, an accurate health history, medication history, allergies, testing as ordered, patient preferences, patient and/or family teaching.

Collects laboratory specimens from in office surgical procedures. Assists the physician with dressing changes and suture/staple removal.

The LPN will demonstrate knowledge of AED use and location.

The LPN will coordinate inventory ordering, stocking, and rotation of stock.

Adheres to Joint Commission standards.

Cleans equipment and workspace. Recognizes equipment malfunctions and refers for repair .

The LPN is engaged in the identification of clinical and/or operational performance improvement opportunities.

Participates in continuous quality improvement initiatives.

Participates in activities to expand knowledge and skills related to professional practice.

Acts as a mentor, preceptor, and instructor in areas of expertise.

Adheres to the policies and procedures as outlined in the exposure control manual: Standard Precautions, Blood Borne Pathogens, Disposal of Sharps, Blood/Body Fluid Spills and Medical Waste Disposal

Adheres to hospital policies and procedures related to:

Mandatory education

Annual Health assessment

Licensure verification

Performs other duties as assigned or required.

Salary Range: $21.22-$30.77 /hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
