**Responsibilities**
**Shift:** Casual Part-Time, Varied Days
**Job Summary:**
Under the direction of the Hospice Clinical Coordinator, the Hospice LPN incorporates a broad spectrum of knowledge in the application of nursing to provide client care in the home or place of residence. The LPN provides care in the home environment based on the IDG ordered plan of care.
**Essential Functions:**
Provides and documents skilled care, treatments and procedures according to the IDG plan of care.
Obtains, reviews and documents written and verbal physician orders, and follows up on orders according to policy.
Assesses and documents skilled nursing needs each visit.
Reports all issues, patient concerns and potential scenarios to Clinical Coordinator or designee immediately for resolution and further direction; follows through on issues that may compromise patient care by using appropriate chain of command.
Participates in call evenings and weekend call and rotation
Assess and identify patient needs for supplies, equipment and teaching materials.
**Qualifications**
**License/Certification:**
Current LPN license issued by the State of Oregon
Possess a valid Oregon Driver's License, automobile insurance and provide one's own transportation.
American Heart Association BLS for Healthcare Providers within 30 days of hire
**Experience:**
2 years acute care or clinical experience
Hospice or home health experience preferred
**Education:**
High school diploma or GED required.
**Pay Range**
$27.70 - $40.17 /hour
