Position Summary:
We have an exciting opportunity to join our team as a LPN - Homecare, Eastern Long Island. In this role, the successful candidate Responsible for using his/her clinical experience to plan the delivery of safe, therapeutic, age appropriate patient care through planning, implementation and evaluation over a scope of practice. With the patient and family central to the care, the LPN incorporates the unique characteristics and needs of each patient and family member into the care delivery process. Works collaboratively within a multidisciplinary health team to promote wellness and optimizes patient outcomes. Performs multiple clinical and clerical duties as required. Complies with established policies, procedures, rules and regulations as they pertain to the hospital as well as federal state and local regulations. Diligently observe all HIPPA guidelines. Performs all other duties as it relates to specialty.

Job Responsibilities:

    • Completes documentation reflecting multidisciplinary coordination. Documents teaching/learning needs of patients and families while on program and in preparation for discharge. Evaluates and documents patient's response to nursing and multidisciplinary team interventions, including patient/family teaching. Evaluates patient's clinical status and attempts satisfactory resolution of patient clinical issues at point of care prior to escalating issues. Takes action to optimize achievement of patient outcomes. Participates in identification, investigation and resolution of actual and/or potential risk management /quality care/ethical issues. Addresses patient issues following established communication hierarchy.
    • Perform other duties as assigned.
    • Identifies patient satisfaction/dissatisfaction with plan of care. If patient expresses dissatisfaction, implements conflict resolution skills and addresses issues with plan of care. Assess and documents patient/family advance care directives and Rights and Responsibilities if indicated. Establishes collaborative and collegial relationships with health care team members.
    • Processes medication orders and refills through the Workflow system. Verifies dosage and medication using various tools progress notes, Clarity, physician or contacting patient if necessary. Enters comments and your initials in Workflow when receiving verbal approval from a physician to refill a prescription.
    • Obtains prior authorization for medication from patient s insurance company as needed Obtains referrals for patient tests ordered by MD as per patient's insurance requirements. Maintains exam room for necessary supplies, cleanliness.
    • Recognizes and responds to issues that may cause problems and/or refers issues to the RN; utilizes the escalation policy. R
