+ JOB REQUIREMENTS

Education

Graduate of a School of Practical Nursing or equivalent practical nurse program.

Experience

Minimum one (1) year nursing experience, three to five years is preferred. Currently licensed as a Practical Nurse in the state of Arkansas. Current CPR certification. Demonstrated knowledge of the principles of growth and development over the life span. Demonstrated knowledge and skills necessary to communicate with the geriatric and adult populations, and to a lesser degree, pediatric populations. Demonstrated ability to think critically, problem-solve and plan effectively. Excellent organization and communication skills are essential. Keyboarding skills required. Must be professional, self-assured, courteous, poised, and versatile. Willing to accept responsibilities, and work with other Home Health team members, patients, providers, referral sources, etc . Requires on the job training to become familiar with department procedures. Basic knowledge of Microsoft Office helpful but not required. Must have reliable transportation, current Arkansas drivers license, and automobile liability insurance.

Normal office environment. Close eye work. Hearing of normal and soft tones. Lifting, carrying up to 35 lbs. Must have the ability to concentrate on fine detail with frequent interruption. Operates computer and standard office equipment, telephones, and scanners. Occasional walking, bending, and climbing, with majority of time in a seated position at a computer.

JOB SUMMARY

Under the supervision of the RN, the person in this position is responsible for utilizing the nursing process to assist in coordinating and managing the care of Home Health patients. Functions as a quality and case management team member in providing and coordinating safe, efficient, and effective Home Health care. They must be knowledgeable and confident in their nursing skills and always operate within their scope of practice. Must be self-assured, self-directed, think critically, and can motivate others toward achieving goals. Able to prioritize and re-prioritize quickly. This position frequently interacts with patients, clinical, and professional staff.
