Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities Duties and responsibilities Incumbent performs retrospective medical review of non-VA health care records to determine medical necessity of visit, differentiating between emergent and non-emergent situations. Reviews medical record documentation for requests for service (RFS) of authorized community care consults to determine RFS indicated medical necessity and urgency. Refers complex cases to the CCRNs for more in-depth review. Analyzes submitted medical documentation to ensure all components necessary to aid in medical record review are present, thereby allowing quick review of inpatient stays by the Clinical Coordinators. Requests additional medical documentation as needed. Using professional standards of care regarding test results determination by elevation of abnormal tests and reporting any deviations from normal to RN or MD/DO. Recognizes deviations from patient's baseline. Demonstrates ability to respond to patient's immediate and long-term needs. Utilizes knowledge of physical, social, and emotional factors when planning patient care, and uses this knowledge effectively in day-to-day interactions with patients and families. Applies knowledge of Medical Center Policies and Procedures to his/her daily clinical functions and responsibilities. Ensures actions and interactions are within the guidelines of VHA and Office of Community Care directives and policies- focusing on patient driven outcomes for Veterans. Using clinical pathways to promote staff involvement in the team environment for patient centered care. Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities. Communicate a plan with the patient and/or caregiver and referring and accepting provider. Operate in an interactive way with all members of the clinical and administrative staff. Perform consult review, consult routing, and completion of consults. Indirect patient care: returning telephone calls, responding to patient emails/inquires, actively managing and renewing prescriptions, reviewing clinical data, communicating normal results to patients, communicating with other VA service departments for technical and equipment needs, and communicating with primary care providers and other referring specialists, as appropriate. Applies the nursing process to systems or processes at the unit/team/work group level to improve care. Demonstrates leadership by involving others in improving care. Identifies and assesses resource utilization and safety issues, taking appropriate action. Acquires knowledge and skills to maintain expertise in area of practice. Participates in educational activities to improve clinical knowledge and enhance role performance. Work Schedule: Monday- Friday 8:00 a. m.- 4:30 p. m. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education - Graduate of a school of practical or vocational nursing approved by the appropriate state agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC). Licensure - Full, Active, Current and Unrestricted license as a licensed practical or vocational nurse in a State, Territory or Commonwealth of the United States or District of Columbia English Language Proficiency - Appointment to direct patient care positions must be proficient in both spoken and written English as required by 38 U. S. C. 7402(d) and 7408(d). Grade Determinations:GS-03 - None beyond the basic requirements. GS-04 - Six months of qualifying experience as LPN or LVN OR Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures OR Graduation from an approved school of at least 24 months duration. GS-05 (a) Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent. (b) Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. [Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/LVNs and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes.
