Summary This LPN position is located within the VA Community Living Center at Central Iowa VA Health Care System, providing care to Veterans receiving extended hospital care and short-term rehabilitation levels of care which includes help with activities of daily living, skilled nursing, and dementia care. The mission of a Community Living Center is to restore each Veteran to his or her highest level of well-being, to prevent declines in health, and to provide comfort at the end of life. Responsibilities The Licensed Practical Nurse assigned to the Community Living Center, is responsible for providing care to a veteran population with widely diverse medical, skilled, mental health and end-of-life needs. He/she is responsible for direct patient care activities under the supervision of the Registered Nurse. Demonstrates excellent customer service skills in communicating with the patient, families, co-workers and interdisciplinary team members. He/she is responsible for direct patient care activities under the supervision of the Registered Nurse (RN). Assignments with supplemental verbal instructions that are given by the RN. Observes, identifies, and responds to patient's need for care, including medication, equipment-assisted care, patient/family education, and oral/written documentation in accordance with policy, procedures, and guidelines. He/she prepares/administers prescribed medication (oral, topical, subcutaneous, and intramuscular), following standard medical center policies. Adheres to the agency requirement for reporting medication errors, reactions, and side effects immediately. - Accurately documents patient information on flow sheets and charts in electronic format. Participates in the interdisciplinary team care planning and delivery of care process. Implements orders and provides care to variable patient population. - Recognizes emergency situations and demonstrates effective action in emergencies. Participates actively in performance improvement measures. Appreciates diversity, keeping an open mind for diversity, cultural competency, and continuous learning. He/she must be poised and articulate in communicating with people of varied educational level and ethnic backgrounds and willing to work cooperatively as a member of a team in every assignment. Other duties as assigned within the scope of practice and/or state licensing board of an LPN. Work Schedule: Sunday-Saturday, Day or Night Shift Available. Day Shift: 7:30am-8:00pm & 7:30am-4:00pm, Night Shift: 11:30pm-8:00am & 7:30pm-8: is a full-time position with a work requirement of 80 hours per biweekly pay period (12 and/or 8 hour shifts). Rotating weekends/holidays are required and occasional off tour rotation. Compressed/Flexible: Available Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: Not Available Virtual: This is not a virtual position. Permanent Change of Station (PCS): Not Authorized Additional Pay: Shift Differential - 10% of basic pay for work performed during regularly scheduled hours between 6pm and 6am. Weekend Premium - 25% of basic pay for work performed between the hours of midnight Friday and midnight Sunday. This includes continuous service connected to work performed during these hours. Holiday Pay - 2 times basic hourly rate for regularly scheduled, non-overtime work on a designated holiday. This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30, 2024.
