**Job Summary**
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Comfort of Home Healthcare team! The ideal candidate will provide attentive, compassionate care to adult clients with developmental disabilities. All services will be provided within the client(s) home(s). This position requires 24/7 availability.
**Responsibilities**
- Administer medications and treatments as prescribed by the physician
- Assist in patient assessments and care planning
- Monitor and record patient vital signs
- Collaborate with the healthcare team to provide comprehensive patient care
- Educate both staff and client(s) on health maintenance and disease prevention
- Maintain accurate patient medical records
- Provide support for patients with memory care needs
- Educate incoming staff on client care requirements during the orientation process
- Lead and instruct Level 1 Medication Aide courses for incoming staff
- Ability to create and implement client care processes
- Providing oversight and guidance to staff and clients
- Care follow-up
- Organization of medical information per client
- Other duties as assigned by Community Registered Nurse (CRN) and Administration
**Skills**
- Ability to proficiently communicate via both written and verbal means of communication
- Proficient in Microsoft Office software (Word, Excel, etc.)
- Proficient in health coaching techniques
- Familiarity with Therap, HRST, or similar electronic health record systems
- Knowledge of anatomy and common medical ailments
- Experience in long-term care and/or behavioral settings is a plus
- Ability to adapt to different healthcare environments
**Employment Requirements**
- Active Licensed Practical Nurse (LPN) license
- Successful completion of a background check including: criminal record, driving record, and abuse/neglect
- Ability to pass a drug screening
- Must have active driver license, proof of auto insurance, and appropriate and reliable means of transportation
- Completion of new hire orientation and all other relevant trainings at onset of employment
Job Type: Full-time
Pay: $65,000.00 per year
