Job Description

Insight Global is looking for a Licensed Practical Nurse (LPN) that participates in providing patient care for one of our largest healthcare providers. The ideal resource will be responsible for the below duties:

-Outreach and scheduling for patients post hospitalization

-Maintaining accurate records of all outreaches

-Location and upload of discharge summaries

-Providing patient education on Medication Adherence

NOTE: This is not a patient facing position. The ideal candidate will be based primarily in an office/cubical setting.

Skills and Requirements

-Current unrestricted Practical Nurse License in applicable state

-Highschool diploma or GED

-Current BLS Certification (if not certified - must acquire certification within 30 days of hire)

-At least 1-2 years of LPN experience in a clinical setting

-Valid driver's license within the state of work

-Knowledge of medical terminology, basic computer literacy, and intermediate Excel skills

-Ability to learn quickly and communicate effectively -HEDIS or Quality Audit experience

-Bilingual (Spanish)

-EMR experience (specifically ECW)

-Experience in geriatrics null
