Job Title: Licensed Practical Nurse

Job Description

We seek a dedicated Licensed Practical Nurse (LPN) with at least 6 months of long-term care (LTC) experience and a valid NYS LPN license held for a minimum of 6 months. The ideal candidate will endorse, interpret, and enforce facility and Nursing Department objectives and procedures. Responsibilities include planning, directing, coordinating, delegating, and evaluating nursing personnel activities, conducting daily rounds, and reviewing resident care plans. You will ensure continuity of care through written and verbal communication, conduct daily conferences with unit staff, and extend courtesy to all unit visitors. Additional duties include coordinating supplies, providing consultation for unit personnel, maintaining records, and assuming responsibilities as delegated by the Supervisor.

Hard Skills

Graduate from an accredited school of nursing

NYS licensure as an LPN or RN

6+ months experience in long-term care

Soft Skills

Good oral and written communication skills

Good interpersonal skills
