Facility Licensed Nurse (LPN/LVN)

Position Overview

The Facility Licensed Nurse (LPN/LVN) is a dynamic and fulfilling full-time role, averaging 36+ hours per week, in our vibrant Pediatric Day Health Centers. As a Facility LPN/LVN, you'll provide direct patient care under the supervision of a Registered Nurse, delivering safe and effective nursing services.

Essential Job Functions:

Monitor and document patient status through ongoing observation and assessment.

Identify patient care priorities to ensure quality nursing care.

Notify the RN of changes in patient status, new physician orders, or changes in the care plan.

Implement physician orders and communicate any changes in patient status to the primary physician.

Document nursing care in progress notes, including patient condition, status changes, communication with physicians, changes in the treatment plan, and patient/family teaching.

Foster and maintain collaborative relationships with the interdisciplinary health team.

Assess family dynamics, strengths, and weaknesses, especially regarding the patient's medical condition and developmental status.

Provide teaching, counseling, and support to families to facilitate adjustment to their child's medical problems and developmental status.

Adhere to company policies and procedures.

Act in accordance with the Nurse Practice Act while complying with company policy and procedure.

Maintain professional, supportive, and responsive interpersonal communication skills.

Participate in case management conferences, staff meetings, and in-services.

Demonstrate initiative, problem-solving abilities, and appropriate nursing judgments.

Stay updated on trends, research, and recent advances in the care of medically complex children.

Assist in the orientation of new employees and act as a resource person.

Maintain awareness of available resources and the process to access them.

Perform other duties as assigned by supervisory staff.

Maintain patient/family confidentiality.

Coordinate activities appropriate to the age group and disabilities based on growth and development.

Administer oxygen and aerosol treatments, monitor intermittent/continuous feedings, administer medications as ordered, start a peripheral IV, supervise meal and snack preparation.

Handle pediatric emergencies and necessary interventions.

Capable of opening and closing the center as needed based on the assigned shift.

Successfully complete annual skills lab, mandatory in-services, and required licensing CEUs.

Requirements:

Current license as a licensed nurse in the state.

No disciplinary board action related to the nursing license.

Knowledge of home ventilators, apnea monitors, oxygen, trachs, trach care, suctioning, special feedings, G-tubes, N/G tubes, J-tubes, and IVs.

Current Basic Life Support Certification.

Evidence of a negative TB skin test or X-ray.

Satisfactory criminal records check.

Knowledge of quality care for medically complex children.

Physical Requirements:

Must be able to speak, write, read, and understand English.

Must be able to walk into the center and ride in a vehicle when transporting patients to and from their residences approximately 50% of the work week.

Perform physical activities, including standing, lifting, or assisting in lifting patients for feeding, changing, and emergency response:

Patients weighing up to 25 pounds - frequently, 80-90% of the workday.

Patients weighing 26 to 49 pounds - occasionally, 20-30% of the workday.

Patients weighing over 50 pounds - with assistance only, 5-10% of the workday.

Must be able to bend, stoop, kneel, squat, and demonstrate flexibility to play, render treatment, rock, hold, or transfer children from beds/cribs to the floor, bathe, and assist in the restroom approximately 90% of the workday.

Must have visual and hearing acuity to monitor lights, sounds, and alarms on applicable equipment, as well as patient pulse and respirations.

Must have sufficient strength and manual dexterity to safely operate equipment, including grasping and manipulating small objects, approximately 80-90% of the workday.

Must be able to discern changes in temperature or texture by touch.

Must be able to respond to unanticipated emergency situations by providing physical support to the suddenly weak or disabled patient, as needed approximately 25% of the work week. Due to the spontaneous nature of children, sudden movements and physical agility may be necessary to protect the patient.

Must be able to function in a wide variety of environments which may involve exposure to latex or other sources of allergic response that cannot always be controlled by the employer.

Environment:

Must be able to function in various environments which may involve exposure to allergens and other conditions.

Possible exposure to blood, bodily fluids, and infectious diseases.
