Job Title: Integrated Case Manager (RN or LMSW)
Job Description
Seeking a dedicated Integrated Case Manager (RN OR LMSW) responsible for the comprehensive management of patient care, including assessment, planning, facilitation, care coordination, evaluation, and advocacy. The role involves creating and implementing discharge plans from an office setting, utilizing a computer to create the discharge of the patient out of the hospital and to their home or the next required medical facility. You will perform comprehensive assessments of patient and family needs, develop personalized care plans, engage in health education, and coordinate transitions between care settings. This position demands critical thinking and professional judgment to assist patients in overcoming barriers, provide counseling on treatment decisions, and advocate for appropriate service delivery.
Hard Skills
RN or LMSW certification
Discharge planning for a patient out of a clinical setting/hospital experience (3+ years)

Work Site
This is a fully on-site position at a hospital in Detroit, Michigan.
Work Environment
Hospital environment within a unit of approximately 20-25 cases at a time. The role is fast-paced, requiring the ability to manage caseloads efficiently to support team dynamics. Standard hours are 8:30 AM to 5:00 PM with a rotating schedule every 4-6th weekend, compensated with time off during the week.
