**Overview**

At St. Luke's, our dedicated team of Hospice RN Liaisons strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.

Nurse Liaisons are a critical role for the success of all post-acute care. A successful liaison will have great people skills, be able to build trusting relationships with hospital staff and providers and have clinical understanding for many acute complex cases that require continued care in the home. **-You will coordinate care and travel within the Twin Falls area.-**

**What you can expect from this role:**

You will be responsible in coordinating end-of-life care for patients with life-limiting illnesses. Much of the Liaison's time is spent by performing assessments and screening, establishing patients' eligibility for care, reviewing referrals for appropriate placement, coordinates discharge and post discharge requirements, meeting with patients/families to describe our services and educate them on the benefits of hospice care. You will also work closely with providers and staff for other possible referrals.

Exempt/SalariedFull Time Days (Mon-Fri) with rotating weekends

Fosters the relationship between patients and the facilities providing their care

Collaborate to design individualized care for adult patients

Represent the organization and participate in marketing and education programs to increase awareness across the system

**Qualifications**

Education: ASN or BSNDegree

Experience: 1 year of RN experience; Home Health, Acute Care and/or Case Management experience helpful

Licenses/Certifications: Current RN License in the State of Practice; current Basic Life Support (BLS) Certification

Valid Driver's License and proof of Auto Insurance
