Job Title: Hospice Coordinator On-Call Registered Nurse
Department: Hospice
Reports to: Hospice Director/Asst. Manager
General Job Description:
The On-Call Registered Nurse serves as a member of the interdisciplinary team (IDT). Provides after-hour patient care that is consistent with the Plan of Care (POC) and is responsible for the provision of services that support the mission, vision, and shared values of Atlantic Health System/Atlantic Visiting Nurse (AVN) while meeting quality outcomes and regulatory standards.
Responsibilities:
Provides after hour on call coverage as scheduled that is consistent with AVN policies and procedures between the hours of 4pm to 8am which includes periods of high volume and down time which equates to a 12.5 work shift. High call volume generally occurs between 4p to 4:30a & decreased call volume between 4:30a to 8am.
Responds to calls from triage in a timely manner and provides patient visits on request of designated triage nurse/Care XM RN
Responds to medical or emotional crisis and/or death as requested by designated triage nurse/Care XM RN
Provides skilled nursing care focusing on pain control and symptom management
Conducts hospice admissions as needed
Consults appropriately with attending physician and medical directors to coordinate patient care and receive orders
Assists RNCMs with their patient assignments by conducting scheduled after-hours visits as necessary to meet patient care needs.
Promotes and maintains respect for privacy, rights, and confidentiality of patients and others.
Reports potential safety concerns and/or incidents to the hospice management team and or IDT members and complete necessary incident reports
Documents care in a timely manner according to AVN policy
Works closely with members of the IDT to provide continuity of care and support the patient and the family and provide updates on changes to patient condition via emailed report
Reinforces care and teaching by making scheduled phone calls for pain and symptom management and other identified needs
Collaborates and coordinates with vendors to obtain DME, supplies, medications when needed after hours
Collaborates and coordinates with hospitals, nursing homes, assisted living facilities to adjust interventions and plan of care as needed
Communicates with other agencies as necessary to coordinate optimal patient care
Educates patients/caregivers on hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary
Attends staff meetings, mandatory in-services, and other meetings upon request
Demonstrates familiarity with and adheres to state/federal regulations, accrediting agency requirements, and AVN policies and procedures
Assumes responsibility for their own personal and professional growth including state licensing/certification requirements
Supervises evening Home Health Aides
Performs other duties as assigned
Field Expectations
Reviews visit documentation including assessments, care plans, medications, POC and IDT notes to familiarize self with patient case as needed
Anticipate being at first scheduled visit at 4pm as the need presents
Pronouncement visits to be done up until 7:00am dependent on location
Reviews email communication to keep abreast of agency communication and respond to patient care questions a minimum of twice/shift
Email pertinent details of admissions and PRN visits by 8am
Knowledge & Skills
Understands and is committed to the hospice philosophy of care
Functions well as a team player
Has a comprehensive knowledge of medication, treatment and therapies for pain and symptom control
Education/Qualifications:
NJ RN License required
BSN preferred
CHPN preferred
Three to five years' experience as a registered nurse in Hospice, Palliative Care, or related field preferred
