Overview: We are seeking a highly organized and detail-oriented Experienced Home Health Intake Coordinator to join our team. The Intake Coordinator is responsible for managing the intake process for new patients, ensuring that all necessary documentation and information are collected and processed efficiently. This role involves coordinating with patients, families, healthcare providers, and insurance companies to facilitate smooth transitions into home care services. Qualifications: * Minimum of two years of experience in a similar role. * In-depth knowledge of Medicare, Medicaid, and commercial insurance. * Strong organizational skills and attention to detail. * Proficiency in using patient management and electronic health record (EHR) systems.
