Your Specific Responsibilities:

The incumbent in this position requires minimal supervision and makes independent decisions in providing direct disease and public health awareness intervention activities to CHD STD Prevention and Control Programs including, but not limited to Partner Counseling and referral Services (PCRS) , epidemiological case analysis and investigations, notification, counseling, and referral services, of individuals exposed to or at risk of infection from chlamydia, gonorrhea, HIV/AIDS, syphilis and other bacterial, biological and viral agents. This position requires firsthand knowledge of public health systems, as well as professional and interpersonal skills to interact with health care clients and health care professionals, and others who impact STD Prevention and Control, Public Health Awareness, and other CHD activities. Duties will also require a knowledge of varied social, economic, cultural, and lifestyle characteristics and a skill in communicating with and eliciting information from patients and target groups with these diverse backgrounds. Minimal guidance and instruction on work methods, procedures and techniques are provided with review of activities at completion for technical adequacy, timeliness, and adherence to instructions and guidelines. All activities are performed observing federal, state, department, CHD information and security confidentiality statutes, rules, regulations, policies, and procedures. The incumbent has access to the medical records department, HMS (EMR) to review charts and document information on the progress notes and in STARS. Completes timesheets in People First and EARS in HMS in a timely manner as directed by DOH. This position primarily covers Alachua County and surrounding areas. Incumbent is responsible for regular communication with STD Manager/Supervisor.

Provides PCRS in accordance with current STD Program policy to a wide variety of individuals infected with or at risk of a STD, HIV/AIDS, or other bacterial, biological and or viral agents to elicit partner and suspect information. Utilizes information for epidemiological analysis to identify the source and spread of cases related to the known infection. Conducts field investigations in accordance with current STD program policy to locate, perform venipuncture, specimen collection, and or refer for counseling, medical evaluation, and or treatment of individuals infected with or suspected to be at risk of infection for STD, HIV/AIDS or other bacterial, biological, and or viral agents.

Performs case management activities as directed in the process of discovering and or identifying individuals identified with a STD, HIV/AIDS, or other bacterial, biological and or viral agents. These activities will include but not be limited to : Work prioritization and records documentation and management , clustering, HIV pre and post test counseling, visual case analysis and epidemiological techniques, STD data entry, health education and or other related activities. This position will also utilize text messages to notify individuals of their need for testing and/or treatment. May transport clients in county vehicles for those that are considered transportation disadvantaged.

Provides technical assistance not considered part of routine duties including, but not limited to: epidemiological assistance to other CHDs or program areas , public health awareness, and emergency response situations.

Visits public and private clinics, clinicians, hospitals, laboratories and other health care facilities to stimulate reporting of cases and positive/reactive test results in a timely manner, (2) provides information on current diagnostic and treatment guidelines, (3) acquaints clinicians with current disease trends and their role in reducing the rate of infection in the community, (4) obtains demographic, medical, or other specific client information including address, telephone number, place of employment, social security number, race , sex, date of birth, test results, diagnosis, treatment, etc.

Assists in the identification, planning, and implementation of targeted screening-outreach programs based on basic analysis of disease trends and distribution among various cultural, social, economic, and subgroup populations in the community.

Responsible for securing the designated information set for the purposes of protecting confidentiality, data integrity, and appropriate access for all information, both confidential and public record, which is stored in hard copy or electronic formats.

Assists program management in training new DIS staff and interns by mentoring trainees in practical applications, techniques, and procedures used to document case management activities, field investigations, reviewing for accuracy, adherence to epidemiological instructions, and completion of epidemiological and non-epidemiological forms.

Completes required records and reports including case management forms, laboratory request, travel reimbursement forms, HMC/CIS, etc. in accordance with current CHD and STD program policy and procedure.

Performs other duties as assigned

Required Knowledge, Skills, and Abilities:

General computer skills, effective interpersonal skills, ability to work independently, ability to draw blood or willingness to learn, a dependable personal vehicle and must be fluent in verbal and written English.

Qualifications:

Minimum - Valid Driver's License, Data Entry Experience

Preferred - associate degree, Phlebotomy experience, Bilingual (English/Spanish)
