The Florida Department of Health (DOH) is responsible for the health and safety of all citizens and visitors to the state. As a public health agency, the Florida Department of Health must monitor the health status of Floridians, diagnose, and investigate health problems, and mobilize local communities to address health-related issues. The Florida Department of Health develops policies and plans that support health goals, enforce laws and regulations that protect the health of all residents and visitors, and link people to needed health care services. The Florida Department of Health also provides specialized assistance to pregnant women, infants, and children with special health care needs, regulates health care practitioners, and provides medical disability determinations.

Title of Position:

Health Educator- ADAP

Your Specific Responsibilities:

Provides outreach, follow-up, and consultation for non-adherence clients with the specific goal of returning for follow-up ADAP appointments and becoming adherence with their medication and the program.

Educates the population about the ADAP program and services with the specific goal and objective of improving adherence, identifying gaps in services to keep them to a minimum and providing guidance of ADAP and case management resources.

Assist as an event coordinator for the ADAP department by promoting social, marketing, education, and resources to the community during regular work hours, as well as, after hours and on weekend.

Plans, manages, coordinates and facilitates health (ADAP) promotion and public health education conferences.

Designs and produces ADAP education media materials, brochures and display for health fairs, seminars, conferences, etc.

Provides ADAP information and serves as a resource specialist to other agencies.

Collaborates with other staff members on development and delivery of health promotion and public health education services to the community.

Acts as a liaison for specific clients dealing with transportation or social issues by delivering medication with a process in place regarding authorization for medications pick-ups.

Acts as a liaison with the Case Management offices by providing guidance in creating a list of clients enrolled in Ryan white Part A who did not follow to enroll in RW Part B.

Act as a liaison during open enrollment to guide clients in the right of path to choose the correct ADAP approved plan marketplace insurance.

Conducts AIDS Drug Assistance and Insurance Premium Plus Programs (ADAP) operations to include, but not limited to, the following: client medical and economic eligibility determinations for enrollment in ADAP and Ryan White (RW) programs and periodic recertifications; coordinating with providers, pharmacies, clinical trials, and other sources of medications.

Monitoring client prescription usage and physician prescribing patterns; and providing information on ADAP and RW services to clients.

Reviews ADAP client files every re-enrollment (six months) to ensure files contain the required documentation for determining client eligibility. Maintains program files, records, and reports within the State requirements for ADAP. Maintains a good working knowledge of the Medicaid program, Medicare program, and private insurance benefit programs; determines assigned benefits and options for client guidance, including assisting and guiding clients with Medicare Part B and D enrollments. Keeps an up-to-date working knowledge, to understand changes, of Health Care Reform and the Affordable Care Act in accordance with HRSA requirements. Acquires and maintains working knowledge of multiple databases and IT programs to include, but not limited to, MS 365 applications, HMS, Provide, e2Hillsborough, CAREWARE, FMMIS, WAGES, and any other State approved software provided to complete the duties within the ADAP Program and the health department. Coordinates client referrals and follow up services as necessary with CBO's and community resources. Initiates and completes COBRA enrollments that will assist Broward Regional in completing documentation to qualified clients for the program. Responsible for entering the Prescription Dispense Authorizations daily into the system for pharmacy operations and for updating the Prescription Main folder with new and active prescriptions.
