The purpose of this position is to promote, maintain and improve individual and community health by assisting individuals and communities to adopt healthy behaviors- specifically behaviors that address sexual health. The Community Health Educator will work with people living with HIV (PLWH) to minimize transmission of the virus, along with at-risk populations to minimize acquisition of HIV and other STIs. The Community Health Educator will achieve this through various testing, networking, counseling, education, and evaluation activities.

JOB DUTIES AND RESPONSIBILITIES:

Conducts HIV and STD testing in the clinic and community in accordance with PA DOH regulations and established Participating Provider Agreement (PPA).

Delivers evidence-based prevention interventions, according to CDC and funding agency guidelines.

Works with Manager to coordinate and conduct outreach, networking, and formation of partnerships with area Community Based Organizations (CBOs).

Participates in social networking and social marketing efforts to engage and recruit highest risk populations for testing and education.

Works with program staff to ensure smooth and efficient linkage to care for newly diagnosed patients.

Develops and presents HIV/STD health education and promotion to patients and community members.

Identifies barriers, develops a treatment plan, and provides assistance and support to patients based on their individual needs relating to health education programs.

Collaborates with Manager to design and conduct evaluations to assess the quality, performance, and effectiveness of the health education programs.

Collaborates with Manager, community health staff, program staff, and community organizations to collect and analyze data to determine community health needs and the availability of services, and to participate in the development of goals for meeting needs.

Provides individual or group counseling and education to patients when necessary.

PHYSICAL AND SENSORY REQUIREMENTS:

Sitting for up to 4 hours per day, up to 2 hours at a time. Stand for up to 6 hours per day, 3 hours at a time. Walk for up to 6 hours per day, 20 minutes at a time. Frequently use hands and fingers. Consistently lift, carry and push objects up to 25 lb. Frequently stoop and bend. Frequently reach above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, and high and low frequencies. Must be able to see as it relates to general, near, far, color, and peripheral vision.

EDUCATION:

Bachelor's degree in human services related field required.

TRAINING AND EXPERIENCE:

At least one year experience working with public health programs required. Previous experience working with low-income and diverse populations preferred. Ability to engage and maintain constructive relationships within a diverse population. Phlebotomy training or willingness to train required. Bilingual in Spanish highly preferred.
