About the "Department"

Montrose Regional Health Case Managers help our patients understand their options concerning the specific situation they are dealing with at the time . We are liaisons between patients and their treatment or care options.

About The Career:

The Care Transitions nurse functions as a member of a multidisciplinary team as an advocate, liaison, and educator for complex patients with high risk for recurrent hospital and Emergency Department admissions. This position supports patients in making a smooth transition and having a successful recovery, post hospital or Emergency Department discharge. The Care Transitions nurse will provide individualized assistance to patients, families, and caregivers to help identify and overcome barriers that may hinder quality and timely medical and psychosocial patient care that contribute to hospital readmissions. The Care Transitions nurse follows the patient starting during hospital admission and for 30 days post hospital discharge. Home and community visits are an expectation of this position.

All About You :

Graduation from an accredited registered nursing program with a minimum of an Associate Degree in Nursing.

BSN preferred.

Current, unrestricted nursing license in the State of Colorado or compact state.

Minimum of two years of experience required.

Nurse navigator experience and/or patient educator is preferred.

BLS required upon hire.
