Using your advanced practice nursing skills, you will assess, coordinate care and provide education for a diverse patient population. This will involve collaborating and consulting with a multi-disciplinary clinical/professional team as well as patients and families to ensure safe and effective coordination of care. You will:
Assess, coordinate, develop and implement individualized care plans
Utilize evidence-based tools for risk stratification
Perform utilization reviews, including authorization reviews
Ensure delivery of concurrent and post-hospital care
Initiate referrals, coordinate resource utilization, and evaluate post-acute care services
Participate in performance improvement projects/processes
Implement a comprehensive DC plan for high-risk patients
Salary Range: $63,800 - $127,000/Annually
Qualifications
We're seeking a self-directed, assertive, creative problem
solver with:

LVN licensure
*
At least three years of acute hospital experience
*
Two or more years of utilization management, case
management or discharge planning experience in an HMO environment
*
Team leading or management experience
*
Systems planning and care management background in a
high-volume setting
*
Proven resourcefulness with strong communication,
diplomacy, organizational, prioritizing, and analytical skills
*
Ability to work effectively and collaboratively with
interdisciplinary teams
*
Computer proficiency
*
Understanding of the functions of a large university
teaching hospital
*
Ability to abstract and interpret medical information from
patient records
*
Familiarity with the laws, rules, and regulations regarding
utilization review and discharge planning of government programs such Medicare,
Medi-Cal, and CCS
*
Ability to develop individualized case management plans
that address physical, vocational, psychosocial, financial, and educational
needs
