**The role you'll contribute:**

This position is responsible for assisting physician/providers by performing Medicare Annual Wellness Visits on eligible patients. This position requires moderate travel throughout the greater Tampa metro area.
**The value you'll bring to the team:**
· Provides care appropriate to adult and geriatric patients, including knowledge of growth and development.
· Performs billable services such as Annual Wellness.
· Performs Medicare Annual Wellness Visit.
**·** Obtains and interprets information to identify patient needs and provide care needed.
**The expertise and experiences you'll need to succeed:**
**Minimum Qualifications Required:**
· Graduate of an accredited school of nursing
· LPN License
**Preferred experience:**
· Minimum of 1 year LPN experience
· RN License
· BLS Certification
