**Monday-Thursday:** 5:00 PM - 8:00 AM
**Friday-Monday:** 5:00 PM - 8:00 AM
**Location:** Coverage from Springfield to Jacksonville

**Your Role as an After-Hours RN:**
**Patient Assessment:** Evaluate patients' physical, psychosocial, and environmental needs.
**Care Planning:** Develop and adjust care plans during each visit.
**Communication:** Coordinate with your clinical supervisor, physicians, and care team.
**Patient Counseling:** Provide safety plans and leverage community resources.
**Care Implementation:** Ensure quality and continuity for your patients.
**Proactive Action:** Identify and address changes in patient conditions.
**Plan Verification:** Confirm the Plan of Care before each visit and deliver care as prescribed.
**Requirements:**
Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
1+ year of experience as a nurse in a clinical care setting.
Home Health/Hospice experience is highly preferred
Current State license as a Registered Nurse.
Ability to travel approximately 50%
Excellent verbal and written communication skills.
Preferred to live in service area
