Job Description: Monday-Friday: 8am-4:30pm.
- This is a professional nursing position which involvesperforming assessments of chronically ill Medicaid clients to receive/maintainhome care services and reside safely in the community. Assessments includereviewing the medical doctor's orders, conducting client visits, completing theUniform Assessment System for New York (UAS-NY), developing care plans,utilizing a time task tool, and creating service orders. RN staff willdocument in the client's case file his or her medical diagnosis, suitable careplan, and determination of the daily/weekly hours of home care servicesrequired and a justification of same.
