Description

This position is for a board eligible or board certified Internal Medicine or Family Medicine Physician in the Oneonta, NY Community-Based Outpatient Clinics for the Albany Stratton VA Medical Center. You will function as a integral member of the Patient-Aligned Care Team and will be expected to work closely with team members, including nursing and administrative staff, in the coordination of patient care.

If selected, you will practice in a Community Based Outpatient Clinic located at the FoxCare Center in Oneonta, NY. You will be assigned a panel of patients to manage under the Department of Veterans Affairs Primary Care Management Model guidelines. Recommended panel size at the division level is calculated based on patient complexity, support staff, and room availability. You will be expected to provide clinical oversight and participate in peer review of other primary care providers through periodic chart reviews.

Your basic responsibilities will include:

  • Providing patient care.
  • Acting as surrogate for absent providers when needed.
  • Attending staff meeting and medical meetings.
  • Collaborating with and supervising mid-level providers.
  • Following Veterans Health Administration guidelines.
  • Timely documentation of encounters.
  • Working closely with Patient-Aligned Care Team (PACT) including nursing and administrative staff .
  • Working within your scope of practice.

Work Schedule: 8:00am to 4:30pm (M-F)
