• Years of experience required: 2 years (Care Coordination/Case management in an acute care setting).
• First time travelers: Not accepted.
• Float: Required, as needed within scope.
• Call: None.
• Weekends: None.
• RTO: Must be approved by manager prior to accepting offer.
• Skillsets required: Acute hospital experience, discharge planning.
• Ratios: 1:26.
• Documentation System: Meditech.
• Scrub Color: Eggplant/darker purple.

Schedule Information
• Holiday Policy 2024 - Allowed (1) holiday RTO request in contract from the following holiday list: Thanksgiving, Black Friday, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day + Travelers can have up to 3 consecutive days off surrounding the holiday but may not include more than one holiday (example: 12/22-12/24 or 1/1-1/3). Each Unit has a maximum about of approvals per holiday - once reached we will be unable to request that date.
