POSITION SUMMARY

  • The Oncology Navigation Coordinator is a registered nurse who coordinates the care of cancer patients across multiple systems from diagnosis to survivorship. Coordination of care begins with the patient at diagnosis and remains throughout the entire diagnostic and treatment process, survivorship and end of life.


    The Navigation Coordinator is responsible for participating in the plan of care through of the promotion of the patient's journey from Pre-diagnosis to the end of active treatment of disease and/or symptoms. This includes being part of all patient interactions that require initiation of care, significant alteration to the plan of care and termination of active treatment. The Navigation Coordinator facilitates necessary consultations to other specialties and takes part in the consults as needed to ensure patient advocacy and consistency in the plan of care.

POSITION QUALIFICATIONS
  • Education and Experience:
    • Associate's degree in nursing required. Bachelor's degree of science/art in nursing required within 3-5 years; Master's degree in Nursing preferred. Oncology Certified Nurse (OCN) required. Two years clinical experience in oncology care required.
  • License/Certificates:
    • Current Minnesota RN license. American Heart Association BLS certification within 30 days of hire. American Heart Association ACLS required within 4 months of hire. Completion of Chemotherapy Certification Course (Cancer Chemo Provider).
  • Special Skills and Aptitudes:
      • Knowledge and skills to implement nursing theory.
      • Demonstrates mature judgment, initiative and proficiency in nursing skills.
      • Effective teaching ability and ability to assess and set learning objectives for orientees and current employees.
      • Strong knowledge regarding the diagnosis and treatment of oncology including but not limited to NCCN guidelines and best practice.
      • Effective communication and professional and positive interpersonal skills.
      • Demonstrates leadership qualities and organizational abilities.
ESSENTIAL RESPONSIBILITIES
  • Oncology Navigation Coordinator:
      1. Serve as the primary patient navigator, guiding patients through the entire cancer care journey and ensuring seamless coordination of services across the oncology department and other specialties. 
      2. Coordinate and manage individualized care plans, ensuring that all aspects of the patient's treatment are integrated and executed according to the plan of care.
      3. Attend all key clinical interactions, including oncology appointments and consultations with other specialists, to ensure continuity and accuracy of the treatment plan.
      4. Act as the main point of contact for patients and their families, providing education on the treatment plan, addressing concerns, and facilitating communication with the healthcare team.
      5. Collaborate closely with oncologists, surgeons, radiologists, and other specialists to develop and adjust care plans as needed, ensuring that all providers are aligned in their approach to patient care.
      6. Monitor and manage the implementation of the care plan, including scheduling appointments, coordinating tests and procedures, and ensuring that all necessary follow-ups are completed.
      7. Guide patients in decision-making regarding their treatment options, ensuring they are fully informed and involved in their care.
      8. Participate in quality improvement initiatives within the oncology department, focusing on enhancing patient navigation, care coordination, and overall patient satisfaction.
      9. Provide support and resources to patients and families throughout the care process, serving as an advocate for their needs and ensuring they have access to all necessary services.
      10. Document and track patient progress, ensuring that the plan of care is consistently followed, and adjusts the plan as needed in collaboration with the healthcare team.
      11. Maintain the ability to both take on the role of the Oncology RN as well as support the roles of other coordinator roles within the Oncology department.
  • RN:
      1. Exercise concern for the patient's welfare, maintains knowledge of the patient's condition, administers medication and performs treatment while practicing high ethical standards.
      2. Provide direct and indirect care of patients assigned to him/her under the guidance of nursing leadership, maintain accurate and complete records of nursing assessment and care, and counsel of the client and family in meeting nursing related needs.
      3. Remain up to date on new nursing skills and technology while utilizing the nursing process (observation, assessment, planning, implementation, and evaluation) in the delivery of care. 
      4. Perform duties in accordance with physician's orders, the Medical Center's policies, methods, and techniques, and in coordinating with other departments and Medical Center personnel. 
      5. Demonstrate Standards of Excellence when other duties are assigned.
