This posting is for the Ventura County Health Care Agency; vacancy location is Ventura and may require travel throughout Ventura County.
POSITION INFORMATION

Reporting to the Director of Pharmacy, the full-time Clinical Coordinator will be responsible for developing, implementing and sustaining clinical pharmacy programs across the hospital. He/she will provide oversight and guidance on medication use quality assurance and compliance with emphasis on patient safety. The Clinical Coordinator:
• Develops and revises policies and procedures to optimize drug utilization and medication safety while meeting accreditation standards and regulations.
• Ensures initial and on-going competency-based assessment of pharmacist's clinical practice.
• Provides leadership for various clinical education programs for students, pharmacists and other healthcare professionals.
Distinguishing Characteristics:

The Clinical Coordinator's primary responsibilities include clinical staff education, and coordination of drug use evaluation (DUE), assists in the development and implementation of new clinical pharmacy services. Works in a multidisciplinary group through committee involvement for creation of clinical practice guidelines and hospital policies. The Clinical Coordinator will also assist in departmental performance improvement, work closely with the medication safety officer on Adverse Drug Reaction monitoring, and compliance with The Joint Commission standards.

PAYROLL TITLE: Pharmacist II

EDUCATIONAL INCENTIVE: Some positions may be eligible for educational incentive. This incentive may be 2.5%, 3.5%, or 5% for incumbents in eligible positions based on completion of an Associate's, Bachelor's, or Master's degree that is not required for the classification.

BILINGUAL INCENTIVE: Some positions may be eligible for bilingual incentive depending on the applicable memorandum of agreement and the needs of the department. In order to qualify for this incentive, incumbents in eligible positions must take and pass the applicable bilingual fluency examination.

Upon recommendation of the Director Health Care Agency, the Director-Human Resources may approve a market-based premium pay of up to $14.00 per hour for Regular Pharmacist II incumbents.

DEPARTMENT/AGENCY: Health Care Agency - VCMC/SPH

Pharmacist II is represented by the Service Employee International Union (SEIU) and is eligible for overtime compensation.

The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-Term), Intermittent, and Extra Help vacancies for this position only, which is a Regular position.

TENTATIVE SCHEDULE

EXAMPLES OF DUTIES
Duties may include but are not limited to the following:
• Identifies and meets staff development needs for the department;
• Develops and presents therapeutic class reviews for medical staff review to include utilization, financial analysis, contract compliance, safety and efficiency;
• Provides oversight on Formulary Review Process for the Pharmacy & Therapeutics committee;
• Directs and coordinates the pharmacy practice rotations with colleges of pharmacy and serves as a preceptor;
• Develops and revises medication related policies and procedures and clinical practice guidelines to assure department meets requirements of Board of Pharmacy, The Joint Commission, and legal regulatory requirements;
• Coordinates Medication Use Evaluations;
• Represents the pharmacy department on medical staff committees to assure rational drug therapy decisions are made in the best interest of the healthcare system;
• Serves as the Consulting Pharmacist for two public health clinics;
• Coordinates and manages pharmacist intervention programs;
• Performs other related duties, as required.

TYPICAL QUALIFICATIONS
Education/Experience:
Requires a pharmaceutical degree, Bachelor's of Science in Pharmacy from an accredited school of pharmacy; and a minimum of twelve (12) months of recent hospital inpatient experience.
