PACE Licensed Practical Nurse (LPN)

Location: Lexington, KY

Schedule: Monday-Friday 8am-4:30pm with occasional on-call, evenings, & holidays

As a BCN PACE Program of All-Inclusive Care for the Elderly team member, you will be a part of a program providing comprehensive health care services for Geriatric participants who can still live safely at the place they call home. Team members who work within our PACE Center can get to know our participants long-term and impact their quality of life. Our PACE experts work collaboratively with an interdisciplinary team to decide what's best for our participants and connect them with all aspects of medical care, social care, and support services, including everyday tasks to PACE participants at home or at the PACE Center. About the role:

The PACE LPN provides direct and indirect nursing care to assigned participants in the PACE Center, home, hospital, nursing home, or ALF. Works under the supervision of the PACE Center Director or Home Care Manager and the Registered Nurse.

Requirements:
  • Graduate of an accredited school of nursing or vocational nurse program.
  • Is currently licensed as an LPN through the State Board of Nursing.
  • Must have at least one year of experience working with the frail and elderly.
  • Food handling permit required, to be completed upon hire.
  • Ability to use equipment with visual and auditory mechanisms.
  • Ability to physically access any home, hospital, nursing home, and ALF (i.e. navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.)
  • Strong communication skills, including face-to-face, telephone, and computer skills.
  • Ability to interact successfully with participants, families, and other agencies to present a positive and professional image.
  • Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.
  • Knowledge of the principles and practice of primary medical care.
  • Demonstrated knowledge and skills necessary to provide care to and communicate with PACE participants.
  • Demonstrated knowledge of the principles of growth and development over the life span.
  • Ability to work collaboratively with PACE staff and volunteers as part of the IDT.
  • Ability to adapt to participant status and needs as directed.
  • Ability to accommodate schedule changes daily due to staffing, participant condition and needs, and new participants.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
  • Employees will be expected to comply with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.




Compensation details: 23.81-29.36 Hourly Wage
