Overview

CHI St. Alexius Health is seeking an Advanced Practice Provider who is passionate about Diabetes Management to work with established patients in Bismarck, ND! Join this highly collaborative, inter-professional, dedicated team which is charged with improving care for patients with diabetes, particularly with respect to transitions of care from the IP setting.

Practice Details:

• Outpatient, clinical position providing care for patients with diabetes
• Monday - Friday schedule; 8:00 am - 5:00 pm; No Call
• EPIC EMR
• Built-in referral base from within CHI St. Alexius
• Provide routine diagnostic, therapeutic and emergency care for patients
• Maintain a caseload of established patients
• Interpret and integrate data in order to appropriately diagnostic and provide therapeutic procedures as needed
• Perform complete physical exams and record findings
• Manage stable chronic disease and refer patients with illness to the appropriate specialty
• Interview and advise patients & family regarding treatment, health, and illness prevention as well as recommending community resources to meet patient & family needs
• Maintain and review patient records, charts and other pertinent information

• Ability to provide exceptional, high quality care
• Support a team-based care delivery model
• Be ready to hit the ground running and continue to grow this busy endocrinology practice
• Preference for candidates with backgrounds in endocrinology, family medicine, or internal medicine

License/Certification:

• Accredited Nurse Practitioner or Physician Assistant
• Board certified with ability to obtain an unrestricted ND license to practice
• CPR certification
• AANP certification

Compensation Information:
$55.29 / Hourly - $55.29 / Hourly
