Job Details
• Case Manager RN position
• Ratio 1:25-30 Adults & Geriatrics
• 5x8hr Days; 830a-5p Mon-Fri plus one full weekend every 4 weeks
• No Call or Holidays
• Hurricane sign-up required but not guaranteed
• Separate remote UR team completes utilization reviews
• In-house case management team ensures diagnostic tests, consults, labs, procedures, etc. are necessary and completed in a timely fashion
• The average caseload is 25-30 patients/day during the week and varies during the weekend (weekend goal is coverage of observation patients and expediting safe discharges)

Job Requirements
• Minimum 3 years Case Management experience required at acute care hospital
• Knowledge of Cerner, CarePort (Allscripts) applications and Powerchart
• Familiarity with documents for Medicare Conditions of Participation
• Ability to function in high paced/high volume atmosphere
• Level 1 Trauma experience preferred
• Degree must be from an accredited school OR have a degree from an institution that is approved by FBON in addition to having 2 yrs recent acute care experience - experience must be consecutive within the past 24 months in a US hospital, per facility requirements
