BC/BE Family Medicine (no OB) - West Greeley
Banner Health, a Top-5 Large Health System and one of the country's premier non-profit health care networks, has an excellent opportunity for a BC/BE Family Medicine Physician to join our multi-specialty practice, recognized by the National Committee for Quality Assurance (NCQA) as a Level-3 Patient-Centered Medical Home, NCQAs highest certification.

Practice details:
- 4-day work week and limited practice call (phone consult only)
- Medical Assistants, network of specialists, and on-site leadership to support an autonomous practice
- Clinical responsibilities include wellness and chronic care disease management, as well as specialized procedures according to candidate's interest
- Located in multi-specialty Summit View Medical Commons in west Greeley (with an incredible panoramic view of the Rockies!). Behavioral Health, Diabetes Education, X-Ray, Lab, Pharmacy, Mammogram, PET, and more on-site
- Salary Range: $235,000 - $265,000 (commensurate with experience), plus incentives
Requirements include:
- Board Certified or Board Eligible in Family Medicine
- Patient-focused, friendly and willing to collaborate in a team environment
