Certification Details
• Certifications must be American Heart Association OR American Red Cross and active upon submission - REQUIRED.

Job Details
• Start Date should be on or very quickly after the Order Start Date listed.
• All facilities the traveler is open to working in should be listed on the submission.
• All shifts the traveler is open to working on should be listed on the submission.
• All departments the traveler is open to working must be checked on the submission cover page.
• All candidates submitted should be preclosed and ready to accept.

Job Requirements
• Minimum 24 months of current OR experience - REQUIRED.
• Travel experience - Preferred.
• Resume with all facilities, facility size, beds, trauma level, and units listed.
• Skills Checklist specific to the discipline (must be less than a year old).
• Active License (verification must be less than 30 days old).

Schedule Information
• RTO should be listed on the cover page, must be less than 7 days.

Additional Details
• Banner will not approve RTO within the first two weeks or holidays.
• No candidates with an AZ home address on their drivers license will be considered - please include a copy in submission.
