St. Luke's regional healthcare organization is seeking a BC/BE Internal Medicine or Family Medicine physician to work alongside a team of 7 NPs to provide long term care and rehab services to our St. Luke's patients in our communities.
General Clinic Information:
St. Luke's remains committed to providing care for our patients who transition to short term rehabilitation, skilled nursing facilities, or assisted living facilities. Our Community Care providers round at the facilities on a regular basis and are able to respond to routine and acute needs. By having the team travel to patients, onsite care can be provided as conveniently and comfortably as possible. Additionally, you can maintain care with your St. Luke's Primary Care Provider while allowing your needs to be met at the facility.
The Community Care Team includes our support staff, comprised of a team of a Business Office Coordinator, Medical Assistant and RN, who work with the facilities and providers so that our team can provide comprehensive care, minimize time spent in the doctor's office and urgent care, as well as decreasing the need for unnecessary ER visits and re-hospitalizations. Facility staff contact us directly which allows for ease of communication and timely responses.

Work M-F supporting patients at facilities in the Duluth, MN, Iron Range, MN and Superior WI areas.
