**Job Title:** PRN Evening Care Manager
**Location:** Addington Place of Shiloh
**Position Type:** PRN (Pro re nata - as needed)
**Job Summary:**
We are seeking a compassionate and detail-oriented PRN Evening Care Manager to join our healthcare team. This part-time position is responsible for overseeing patient care during evening hours, ensuring the delivery of high-quality services to patients and their families. The Evening Care Manager will collaborate with interdisciplinary teams to assess patient needs, develop care plans, and monitor patient progress, ensuring that all care is aligned with organizational standards and policies.
**Key Responsibilities:**
- Assess patient needs and develop individualized care plans, in collaboration with healthcare providers and family members.
- Implement and oversee care plans, ensuring patients receive the necessary support and interventions during evening shifts.
- Monitor and evaluate patient progress, making adjustments to care plans as necessary.
- Serve as a point of contact for patients and families, providing education and support regarding care options and resources.
- Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and therapists, to ensure comprehensive patient care.
- Maintain accurate and timely documentation of patient assessments, care plans, and progress notes in compliance with company policies and regulatory requirements.
- Provide mentorship and guidance to nursing staff and other team members, fostering a supportive and collaborative work environment.
- Participate in training and development initiatives to remain current with best practices and changes in the healthcare industry.
- Ensure that all patient care activities adhere to ethical standards and confidentiality regulations.
**Qualifications:**
- Bachelor's degree in Nursing, Social Work, or a related field; Master's degree preferred.
- Current and valid nursing license (if applicable) or relevant certification in care management.
- Previous experience in healthcare management, case management, or a related role is preferred.
- Strong understanding of patient care best practices and regulatory requirements.
- Excellent interpersonal and communication skills, with the ability to build rapport with patients and families.
- Strong problem-solving skills and the ability to work independently in a fast-paced environment.
- Proficient in electronic medical record (EMR) systems and Microsoft Office Suite.
**Work Conditions:**
- Evening shifts, including weekends and holidays as needed.
- May require standing for long periods, flexibility in schedule, and the ability to handle multiple priorities in a busy environment.
