Online Orientation Acknowledgment- Facility will provide the traveler with a link to do the online orientation
Badge Photo - A JPG head shot picture of candidate is required prior to start
Exemptions - Candidate will need to complete Facility exemption form. If requesting religious exemption, a personal statement must be included with exemption. For medical exemption, must provide supporting documentation from a physician
